Vaccine, Forms, and Medical Records

Vaccine Records- at least 1 business day to process, must be requested over the phone or in person.
School Forms- at least 3 business days to process, must be requested over the phone or in person, child must be up to date on their annual physical in order to complete.
Medical Records- Medical Release of Information must be completed in office or faxed to us from the requesting party in order for them to be released.
These documents cannot be requested via our website as it is a Patient Privacy concern.

Medication Refills

We do not accept medication refill requests from local pharmacies via fax or phone.
The Parent/Guardian must call and request medical refills from our office directly. These are completed in 24-48 hours.
Certain medication refills may require an appointment within at least 3 months of a visit.

Scheduling/Canceling/Rescheduling Appointments

Please call our office directly with these requests. We cannot exchange patient information through this website or email.

Referral Requests

Referral requests require that the patient be up to date on their annual physical.

If the referral request is a new concern, the provider may request that the patient be seen in the office before a referral is made.

This is our office policy due to varying insurance policy coverages, requirements for authorization, and internal processes we must provide.
